The industry moved dispensing away from the counter. We think that was the mistake.

Two things happened to retail pharmacy at once. The chains got large enough that the work around a prescription stopped being worth doing, and the manufacturers built hubs that do that work beautifully for exactly one product and send the rest of the patient's list somewhere else.

What fell through the gap is the thing that decides whether a therapy works: somebody who knows the whole medication list, will argue with a plan, and will notice when a patient stops refilling. OnPoint is that, at scale, with the operations of a hub behind a counter a patient can walk up to.

A prescriber base that came from the work, not from a rep

Practices route to us because a prior authorization stopped coming back to their desk. That is expensive for a competitor to replicate and slow for a competitor to unwind.

Ten physical stores inside the densest patient market in the country

Mail-only competitors cannot hand a post-operative regimen across a counter on the morning of surgery. Chains in the same footprint will not do the access work.

Depth in the specialty with the worst access friction

Ophthalmology carries more prior authorizations and step-therapy requirements per prescription than almost anything in medicine. It is the hardest place to operate and therefore the least contested.

Owners behind the counter.

A pharmacist with equity in the store answers the phone differently, and stays. In a sector where pharmacist turnover is the binding constraint on growth, that is the whole game.
