Have your prescription card ready
We need the plan name, member or subscriber ID, RxBIN, RxPCN and RxGroup or group ID. Bring both sides of the card, including a separate prescription card and any secondary coverage.
Not every card lists every field. Your pharmacy can help identify what is needed. Do not send health information to a general contact inbox.

A plan name or PBM logo does not establish that every OnPoint location is in your network. Ask about your exact plan, any preferred or required specialty pharmacy, and your prescription before transferring.
We will discuss the expected cost before dispensing. Benefits, deductibles and prices can change.
When a plan needs more information
We help identify coverage requirements and coordinate with your prescriber's office. Your prescriber supplies the clinical information, and the request is submitted through the process your plan permits. The plan decides whether to approve coverage.
How prior authorization support worksHelp with costs
Ask your pharmacist to review available payment and assistance options. Eligibility, program terms and funding determine what can be used. Savings and approval are not guaranteed.
Manufacturer copay coupons generally cannot be used with prescriptions paid for by Medicare, Medicaid or other federal health care programs. Independent assistance programs have separate rules.
Discount cards are generally used instead of insurance for a purchase, not combined with it. Ask how a cash or discount purchase affects your plan deductible and out-of-pocket totals before choosing.
I have New York Medicaid. What should I bring?
Bring your current Medicaid and prescription benefit information. Your pharmacy will check eligibility, the applicable pharmacy benefit and its participation for your prescription. NYRx rules differ from commercial plans.
Can you check Medicare Part D or EPIC?
Call your chosen location with your exact plan details. Participation and any coordination with EPIC must be checked for your plan, prescription and pharmacy.
What if my insurance changes?
Send the new card details through the refill form, or call your store or bring your card in person.
What if I do not have insurance?
You can still call to ask about prescription pricing and any assistance you may qualify for. Your pharmacy will explain the available options.
